First Visit

You will need to present your Insurance Card, Photo ID and a method of payment at your first visit. We accept Visa, MasterCard, Discover, Checks and Cash. Your office co-pay is due at check-in and before your visit.

Our office accepts Medicare and most commercial insurances and we are contracted with the Santa Clara County IPA HMO as well as CareMore HMO. Please make sure you verify your network providers list with your insurance before making an appointment. Also, it is the patient's responsibility to verify their insurance plan, eligibility, coverage and any deductible and out of pocket dues prior to making an appointment.

We will bill your insurance on your behalf and we will send you statements with details of all charges and your financial responsibility. Our billing company will follow up with patients to ensure that all accounts are up to date.

You are also required to complete and sign all registration forms and send them in before your visit or bring them in with you. Failure to present completed forms can result in delays for your visit and we reserve the right to reschedule your visit. You can print the forms from this website or provide us with an email address to gain access to our patient portal and complete the forms online.

If you need assistance with translation from English, physical assistance for patients in wheelchairs, please make sure you arrange for a family member or a care giver to come to your appointment with you.

Payment Information

Our office accepts Personal Checks (deposited via an in office electronic check scanner), Cash, Visa, Master Card and Discover.

All insurance co-pays and any over due balances need to be paid at check-in and before your visit. Patients can make payments in person, by phone or by using our Patient Portal.
